That is not necessarily a bug, neither rare. Minecraft skin files are just flat pngs, and when Notch created his skin Minecraft used to read skin files by selecting certain pixels to show on the player's 3D model, and ignoring any other pixels in the image that are outside of the area it is programmed to read and put into a skin. Notch's skin, differently from most skins, has black pixels in the areas that Minecraft should ignore them. But when the game updated the skins to add layering, these regions that the skin reader originally ignored, were used to draw the layers of a skin, and because of that, the black pixels on Notch's skin began to be read by the game as layers instead of being ignored. So you can not only replicate that easily nowadays, but it also isn't even a bug, it's just a mistake of Notch.
